For admission to Ashlan Village the resident and/or responsible party will need to do the following prior to moving in.
1. Reserve a room with a $500.00 deposit. Once the resident moves into Ashlan Village the deposit becomes the admission fee.
2. Take the Physician statement, obtained from Ashlan Village staff, to his/her primary physician and ask them to complete the form.
3. Obtain an up-to-date Tuberculosis skin test (2 step) or chest x-ray. Any Prior TB test must be with completed within the last 30 days. This can be given at Ashlan Village by our nurse Chrystal Bass.
4. You must be assessed by Chrystal Bass RCC at Ashlan Village or at the resident's current location.
5. Complete all admission paperwork including the Residency agreement and Payment agreement prior to moving into Ashlan Village.
